Glaucoma Passport

A new concept in patient engagement is the Glaucoma Passport. This is a clinical tool designed by Professor Peter Shah and Dr Freda Sii to enhance the management of glaucoma patients by education and involving them in their own care.  The need for such a resource came about from many hours of discussion with patients from all sections of the community and active practitioners involved in care of glaucoma patients.  The passport contains information critical to the patient including sections on their eye history, medications, adverse reactions, drop instillation and contact numbers in case of emergency and sources of further information.  The passport is designed for patient empowerment and by involving the patient in their own care,  results in improved compliance and ultimately better visual preservation.

Professor Peter Shah is a Consultant Ophthalmic Surgeon and Supra-Regional Glaucoma Specialist at University Hospitals Birmingham NHS Trust. He is also a Visiting Professor to the NIHR Biomedical Research Centre (Moorfields Eye Hospital and UCL Institute of Ophthalmology). He is Honorary Professor of Glaucoma at the Centre for Health and Social Care Improvement at the University of Wolverhampton.
